ABSTRACT

Objective:To investigate the efficacy and safety of a novel modified Blumgart pancreaticojejunostomy in laparoscopic pancreaticoduodenectomy (LPD).Methods:Between May 2021 and January 2022, 13 successive cases from Lihuili Hospital Affiliated to Ningbo University who underwent LPD were enrolled in this retrospective study. The study retrospectively analyzed the demographic characteristics, perioperative outcomes, and pathological results of these cases.Results:Twenty patients underwent LPD success-fully and one required conversion to open surgery. The operative time was (308.6 ± 61.7) min. The duration for PJ was (26.7 ± 4.3) min. The estimated blood loss was (188.1 ± 94.2) ml. The postoperative hospital stay was (14.2 ± 3.5) d. There was one case of biochemical leakage and no case of grade B or grade C pancreatic fistula.Conclusions:The new method is safe, simple and feasible. The novel method could reduce the incidence of pancreatic fistula and other complications after LPD.

ABSTRACT

Objective To evaluate the differential diagnosis value of shear wave elastography for Hashimoto's disease with benign or malignant thyroid nodules.Methods Fifty four patients of Hashimoto's disease confirmed by biopsy or pathology were enrolled.Based on benign or malignant of nodules,patients were divided into benign nodules group and malignant nodules group.And the benign group were divided into true nodules and sham nodules.Young's modulus values were measured and compared among different groups.The ROC curve was drawn to evaluate the diagnostic efficency.Results There were tatol 162 nodules in 54 cases with Hashimoto's disease,including 131 of benign nodules (93 of true nodules,38 of sham nodules) and 31 of malignant nodules.The values of Young's modulus for benign,malignant,ture nodules and sham nodules were (33.04±10.95)kPa,(66.15±19.17)kPa,(35.95±9.74)kPa and (30.13±11.87)kPa.For mean values of Young's modulus,there was significantly statistical difference between benign and malignant nodules (t=12.91,P<0.01).There was significant statistical difference of the Young's modulus among sham nodule group,true nodule group and malignant nodule group (F 36.20,P<0.01).Compared with the other two groups,the value of Young's modulus in malignant nodules was significantly higher than that in the other two groups (both P<0.01).Taking 43.65 kPa as the diagnostic point,the sensitivity,specificity,positive likelihood ratio,and negative likelihood ratio of Youngs modulus in diagnosis of benign and malignat nodules were 90.63%,81.25%,4.83,and 0.12.Conclusion Shear wave elastography technique can quantitative analysis the Hashimoto's disease and complicated nodules,which is helpful to determine the essence of the nodules.

ABSTRACT

Objective To explore the value of static and dynamic MRI before and after operation of pelvic organ prolapse (POP). Methods 29 patients with POP (POP group)and 12 normal women (control group)underwent static and dynamic MRI.The morphologic changes of pelvic floor were observed on MR images.The measurements of bladder,uterus,Douglas pouch to pubococcygeal line (B-PCL,U-PCL,D-PCL),the puborectal hiatus line (H-line),muscular pelvic floor descent (M-line),the levator hiatus size (LHS),the levator plate angle (LPA),the iliococcygeus angle (ICA)and the urethral inclination angle (UA)were recorded on dynamic MR images.Results 19 cystoceles,28 uterine prolapses,4 rectoceles and 14 hernias of Douglas pouch were detected with MRI.29 cases of pelvic floor relaxation,27 cases of levator ani muscle defect and 24 cases of pubocervical fascial defect were found.The values of B-PCL,U-PCL, D-PCL,H-line,M-line,LHS,LPA,ICA and UA of POP group were larger than control group (P<0.01).The positions of pelvic organ returned to normal in 9 cases of 21 postoperative cases,while 12 cases remained prolapses.There was no displacement of mesh in 8 cases of mesh implant.The values of B-PCL,U-PCL,D-PCL,UA after operation were smaller than those before operation (P<0.05).Conclusion Static and dynamic MRI can evaluate morphological and functional changes of pelvic floor before and after operation of POP comprehensively,and may reveal those invisible pelvic floor dysfunction and postoperative remnant defects.

ABSTRACT

To describe the imaging features of gastrointestinal stromal tumors [GISTs] at initial presentation with clinical, surgical, and pathologic correlation, and to evaluate values of various techniques in GISTs. This retrospective study recruited 70 patients with histologically proved GISTs between December 2004, and May 2009 in the Department of General Surgery, Zhongshan Hospital, Fudan Univeristy, Shanghai, China. Each patient underwent CT scanning, 39 patients underwent simultaneous endoscopy, 12 patients underwent endoscopic ultrasound [EUS], and 36 patients underwent transabdominal ultrasonography [TAUS] simultaneously. Features of GISTs were assessed. Computerized tomography findings showed an eccentric mass in 44 patients, an intraluminal component in 24, and a transmural distribution in 2. Forty-two tumors were dumbbell-shaped, 2 were round, while 26 were irregular. Forty-three tumors presented with well-defined masses, while 27 with unclear borders. The arterial phase attenuation showed the continuous enhancement. The portal-venous phase attenuation was heterogeneous in 26 and homogeneous in the other 44. There was a significant correlation between certain CT features and tumor risk stratification. Gastrointestinal stromal tumors were characterized by a smooth shape and normal overlying mucosa in endoscopy, hypoechoic, and solid in TAUS. Imaging examinations are pivotal in the management of GISTs. The CT scan is valuable in the diagnosis, staging, and treatment planning of GISTs. Endoscopy and EUS contribute to the detection of mucosal lesions. Other methods including TAUS, fluorodeoxyglucose positron emission tomography, CT gastrography, and MRI help in specific cases

ABSTRACT

<p><b>OBJECTIVE</b>To assess the value of magnetic resonance (MR) myocardial perfusion imaging (MRMPI) in evaluating the myocardial viability in patients with myocardial infarction.</p><p><b>METHODS</b>MRMPI was performed in 51 patients with myocardial infarction using a 1.5 T MR scanner. All the patients were examined using IR-turbo FLASH sequence during the first-pass and delayed phase 5-30 min after injection of 0.1 mmol/kg Gd-DTPA at the rate of 4 ml/s. The short axis images were acquired during the first-pass, and both the short axis and long axis images were obtained during the delayed phase. The left ventricular wall on the short-axis slice was divided into 8 segments. A correlative study of the results of the rest and stress (99m)Tc single photon emission computed tomography (SPECT) was carried out in 21 patients.</p><p><b>RESULTS</b>In the 51 patients with myocardial infarction, 42(82.3%) showed hypoperfusion during the first-pass imaging and 50(98%) had delayed hyperenhancement. In the 21 patients receiving SPECT, 48 nonviable segments was detected among the 168 segments scanned by (99m)TcSPECT, and MRMPI showed delayed hyperenhancement in all the infracted areas. Of the 120 viable segments detected by rest and stress (99m)Tc SPECT, 97 segments (80.8%) were found to be free of delayed hyperenhancement by MRMPI. With the rest and stress (99m)Tc SPECT as the reference, the sensitivity and the specificity of MRMPI were 100.0% and 80.8%, respectively.</p><p><b>CONCLUSION</b>MRMPI allows effective identification of the myocardial viability and nonviability as well as the severity and extent of the myocardial infraction.</p>

ABSTRACT

OBJECTIVE@#To investigate the application of CT hepatic volume measurement combined with CT perfusion imaging (CTPI) by Spiral CT in evaluating the hepatic functional reserve in cirrhosis.@*METHODS@#CT volume measurement of livers was performed in 32 patients with cirrhosis and 20 patients with normal livers. CTPI had been taken in 25 patients with cirrhosis and 20 patients with normal livers. The hepatic volume, parameter of blood flow perfusion, and liver volume-perfusion index were observed and analyzed. Correlations between graded fractions of hepatic function and liver volume, parameter of perfusion, and liver volume-perfusion index were also analyzed.@*RESULTS@#The values of liver volumes (LV, LVs) reduced gradually (P<0.01) among the groups of hepatic function Grade A, B, and C. The values of the hepatic portal perfusion (HPP), total liver perfusion (TLP), hepatic arterial perfusion index (HAI), and portal perfusion index (PPI) of the cirrhosis group were significantly lower than those of the normal ones (P<0.01). The values of the HPP reduced gradually among Grade A, B, and C (P<0.05 or P<0.01). The values of PPI reduced significantly between Grade C and Grade A, B (P<0.05 or P<0.01). The liver volume-perfusion indexes (VPI, VPIs, VPPI, VPPIs) reduced gradually among Grade A, B, and C (P<0.05 or P<0.01). The correlation between liver volume-perfusion indexes and graded fractions of hepatic function was higher than other indexes, among which the correlation of VPPIs was the highest.@*CONCLUSION@#Changes of volume and blood flow perfusion of the liver are related to the clinical grade of hepatic function. The CT volume measurement combined with CTPI will be a more comprehensive way in evaluating hepatic functional reserve.

ABSTRACT

Objective To investigate clinical and MRI features of the reversible posterior encephalopathy syndrome (RPES).Methods Clinical and MRI materials of 4 RPES cases confirmed by integrated clinical diagnosis were retrospectively analyzed.Results All the cases presented with a rise of blood pressure.The neural symptom include severe headache (2 cases),dysphoria (3 cases),blur of vision (2 cases),impairment of impermanent memory (1 case),seizure (2 cases).In MRI,the lesions mainly systematically located in posterior parietal lobe and occipital lobe in all cases,or randomly located in frontal lobe (2 cases).Lesions demonstrated high signal on T_2 weighed and water-suppressed sequences in MRI.The lesions dominantly located in cortex in 2 cases,in both cortex and subcortieal white matter in 1 case and in subcortical white matter in 1 case.Four cases resolved their lesions when re-examined. Conclusions RPES represents such characteristic MRI findings as symmetrical distribution of reversible lesions in posterior brain with involvement of cortex or subcortieal white matter.When combined with hypertension and neural symptoms caused by involvement of posterior brain structure,it is helpful for early differential diagnosis of this syndrome.

ABSTRACT

<p><b>OBJECTIVES</b>To screen and clone the genes encoding hepatocellular carcinoma associated tumor antigens.</p><p><b>METHODS</b>A hepatocellular carcinoma cDNA express library was constructed with ZAP vector and analyzed by serological analysis of recombinant cDNA expression library (SEREX) with sera from autologous and allogenous patients. Monoclonalized positive phage clones were converted into pBK-CMV phagemid forms by in vivo excision. The cDNA inserts were determined by restriction endonuclease digestion with EcoR I and Xho I. The cDNA inserts were sequenced and analyzed with bioinformatics. LIMS1 insert was cut from the clone HCL5-70 and constructed into pQE 31 express vector. The recombinant LIMS1 was expressed in M15 and analyzed with SDS-PAGE and Western blot.</p><p><b>RESULTS</b>Fourteen genes were cloned from autologous screening and eleven genes were obtained with allogeneous analysis. One gene, kinectin, was identified in both autologous and allogeneous screening. Eight of the total twenty-four genes were unknown for their functions; the other sixteen genes can be classified into eight groups according to their established or putative function. Recombinant LIMS1 was expressed in M15.</p><p><b>CONCLUSION</b>The identification of hepatocellular carcinoma associated tumor antigens provides potential targets for immunotherapy of hepatocellular carcinoma patients and will help in the understanding of the carcinogenesis of hepatocellular carcinoma.</p>
